This video demonstrates a mitral valve replacement of a rheumatic mitral valve through a minimally invasive approach in a patient with a prior Y-Incision aortic annular enlargement. Techniques highlighted in this case include excision of the mitral valve leaflets and thickened chords, papillary muscle approximation, and placement of a bioprosthetic mitral valve. This case illustrates a rare instance of progression of mixed rheumatic mitral stenosis and regurgitation arising years after aortic root enlargement with Y-incision.
