Case 189: Manual of PCI - Left main thrombus

A patient with prior aortic valve replacement and single vessel CABG with SVG to the right PDA presented with NSTEMI and was found to have a filling defect in the left main trifurcation. He was turned down for CABG and referred for complex PCI. His wedge pressure was 11 mmHg and he had severe peripheral arterial disease, hence hemodynamic support was not used during PCI. The LAD, ramus, and circumflex were all wires followed by thrombus aspiration using the Penumbra Cat Rx catheter with successful retrieval of a large thrombus. OCT showed thrombus in the mid circumflex that was successfully stented. Provisional stenting was then done from the left main into the LAD that was complicated by slow flow. After intracoronary administration of nicardipine and cangrelor TIMI 3 flow was restored.
