Patient with Chronic Stable Angina, Drug Defaulter since 2 years- previously stented i LAD and LCx during AWMI few years ago.
Also, Diabetic.
Patient has TVD on Check Angio.
Was advised CABG at a different hospital, however, patient did not want to get it done.
RCA was giving Grade I,II Collaterals to LCx surprisingly.
Patient also had Ayushman Bharat PM-JAY card - hence, limitations were there as far as IVUS/OCT Imaging use etc..
Patient was adamant not to get CABG done, and arrived to our hospital for PTCA and Stenting.
we explained need for CABG again due to Diabetic status and diffuse disease, and also had a CTVS surgeon discussion held, however, patient did not budge- in the end, we took PTCA consent and CABG refusal by patient and did a very good Angioplasty and Stenting To LAD and LCX. RCA to be done as a staged procedure after 1-2 months to limit contrast and radiation exposure.
Patient was discharged after 48 hours with relief in Angina.
All CABG need not undergo bypass surgery, some may do well with PTCA and Stenting also. However clear information should be relayed to them without bias and allow them to make a choice, guiding them in their doubts, especially in cases of CSA.
