Leriche syndrome, also known as aortoiliac occlusive disease, is the blockage of the distal aorta and both common iliac arteries, typically seen in chronic smokers, elderly individuals, and patients with thrombotic disorders, coronary artery disease, or heart clots. It presents with a classic triad of symptoms: gluteal and thigh claudication (pain while walking), absent bilateral femoral pulses, and erectile dysfunction. Diagnosis is primarily clinical and confirmed through a CT peripheral angiogram to determine the site and extent of the occlusion. Treatment depends on the length of the blockage: short-segment occlusions can be managed with minimally invasive bilateral iliac angioplasty and “kissing” stent placement, allowing same-day mobilization and discharge by day two; long-segment occlusions require open surgical aortofemoral or aortobifemoral bypass using a Y-shaped prosthetic graft. Postoperative recovery typically involves 1 day in the ICU and discharge by the 4th or 5th day. Prognosis is favorable, especially in patients who stop smoking and maintain good overall health, with grafts lasting up to 20 years.
