Large condyloma Acuminata - managed by surgical excision and fulgration. Patient was a 40 years old man who got HPV infection. The patient tested negative for other Sexually transmitted diseases. Since the disease load was large, it was decided to do surgical excision of warts and fulgration of the base. The process was done under local anaesthesia. The process of removal of the warts a tedious process, each and every warts was individually removed and base fulgrated. The warts were circumferential around the anal canal opening. I tried to keep the excision of the warts limited to the superficial layer so that patient does not develop anal stenosis or anal stricture late on. After removal of the warts the anal canal opening became visible, luckily for the patient the anal canal was not involved and there were no intra-anal warts. After removal of the warts, the wound was cleaned with betadine and T - bandage dressing was done for 24 hours. The patient was started on Imiquimod cream for 4 months. And also advised vaccination to prevent further infections of different serotypes of HPV. In this case it was HPV serotype 11 that was tested. The patient recovered without any complications. There was no stenosis or anal stricture. The skin healed satisfactorily though there was minor pigmentation of the skin but it also became normal after 2 months.
HPV is one of the most common sexually transmitted disease and it is advised in case someone gets the infection, he or she should immediately seek medical help rather than try to hide it and let it grow to such large size.
