It entails a proposal to use detailed, pre-composed pre-operative notes that act as both a briefing script for the surgical team and, once updated after surgery, the official operative report. By circulating these notes in advance and rehearsing the plan together, surgeons and the MDT improve communication, preparedness, and safety—particularly in complex paediatric cardiac procedures—while also producing more accurate documentation and streamlining postoperative record-keeping.
