Most heart failure readmissions aren’t because we lack good treatments.
They happen because of small, fixable management errors:

Timid diuresis
Poor monitoring
Unnecessary GDMT holds
Discharging before true euvolemia
Weak IV-to-PO transitions
Inadequate discharge education and follow-up
