Penile curvature after radical prostatectomy is an under-discussed complication of prostate cancer surgery. While many patients are aware of the risk of erectile dysfunction following surgery, few know about the connection to Peyronie’s disease or other conditions like Kelami syndrome. Research shows that up to 16% of men develop Peyronie’s disease within three years of prostate cancer surgery, yet this complication is often not discussed by doctors or understood by patients.
Dr. med. Dr. phil. Stefan Buntrock is a board-certified urologist and specialist in sexual health from Göttingen. He dives into the scientific literature to shed light on why this happens. Factors such as nerve-sparing techniques, postoperative erectile dysfunction, and fibrosis of the Tunica albuginea are significant contributors. Additionally, trauma to the penis during surgery—such as catheter use and bladder sutures—can further increase the risk.
Interestingly, not all cases of penile curvature involve Peyronie’s disease. My clinical experience and a key study suggest that some cases may lack the characteristic palpable plaques, raising questions about whether this could be due to Kelami syndrome, a condition involving scarring of the Corpus spongiosum.
