Hi I'm Prerak Juthani - an internal medicine resident at Stanford! I am here to discuss the fundamentals of Heart Failure! Heart failure (HF) is a complex clinical syndrome characterized by symptoms and signs resulting from any structural or functional impairment of ventricular filling or ejection of blood. It is typically manifested by dyspnea, fatigue, exertional intolerance, and fluid retention. This video will summarize the nuances of Heart Failure for all medical students and early stage residents. 03:12-05:00 - What is the Classic Presentation of Heart Failure?
- The Heart Failure Society of America defines HF as a syndrome caused by cardiac dysfunction, generally resulting from myocardial muscle dysfunction or loss, and characterized by left ventricular dilation or hypertrophy. This dysfunction can be primarily systolic, diastolic, or mixed, leading to neurohormonal and circulatory abnormalities.
HF can be classified based on left ventricular ejection fraction (LVEF) into:
• Heart failure with reduced ejection fraction (HFrEF): LVEF ≤ 40%
• Heart failure with mildly reduced ejection fraction (HFmrEF): LVEF 41-49%
• Heart failure with preserved ejection fraction (HFpEF): LVEF ≥ 50%
• Heart failure with improved ejection fraction (HFimpEF): Baseline LVEF less than 40%, with a ≥10-point increase from baseline and a second measurement of LVEF greater 40%.
