Phase III trial comparing neoadjuvant mFOLFIRINOX versus PAXG in stage I-III PDAC

Michele Reni, MD, IRCCS San Raffaele Scientific Institute, Milan, Italy, comments on the results of a randomized Phase III trial (NCT04793932) comparing the modified FOLFIRINOX regimen with the PAXG regimen for stage I-III pancreatic ductal adenocarcinoma (PDAC), highlighting that the PAXG regimen significantly prolongs event-free survival (EFS), with a EFS increase of almost six months and a three-year EFS rate more than doubling from 13% to 31%. Prof. Reni suggests that this new regimen may be considered a new standard for PDAC, despite concerns about overall survival (OS) data, and proposes that a maintenance therapy trial after surgery could be a promising strategy to further improve three-year EFS rates. This interview took place during the 2025 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.
