Many patients and families have fears about chemotherapy due to side effects. Hearing about the miraculous results of immunotherapy, like tumor regression in rectal cancer or increased survival in stage four cancer, creates hope that it is the safest and most effective option. But is this true? Does immunotherapy work the same way for every patient?

Dr. Praveen Kammar explains that immunotherapy works by enabling the immune system to recognize and fight tumors. Understanding how immunotherapy works against cancer depends on three key factors

1. Tumor Visibility
The immune system can only target tumors with specific molecules, called tumor visibility. Cancer cells may express receptors or proteins like Her2Neu in breast and stomach cancer, MSI High in colorectal cancer, or PDL1 status. Only when these immunotherapy cancer treatment targets exist can immunotherapy work. Not all cancers have suitable targets

2. Scientific Evidence
Presence of a target does not guarantee effectiveness. Strong scientific evidence and clinical trial results are required before recommending immunotherapy for a specific cancer. Next-generation sequencing may show multiple targets, but effectiveness must be proven. Understanding how immunotherapy is done and how immunotherapy is given relies on solid scientific data

3. Patient Overall Health
Immunotherapy often has fewer side effects than chemotherapy, but it is not without risks. Immunotherapy side effects must be considered and patients must be healthy enough to sustain treatment

In short, immunotherapy eligibility criteria depend on tumor targets, scientific evidence, and patient health. When all three align, immunotherapy can be recommended
