Ovarian Cancer या TB? | Confusing Case Solved by Dr Praveen Kammar | Cancer Surgeon, Mumbai
What was the patient’s story?
A 41 year old premenopausal woman came with lower abdominal pain, indigestion, mild weight loss and evening fever for 2 to 3 months. Her ultrasound and CT scan showed an ovarian mass, some fluid in the abdomen and nodules on the peritoneum. The first impression looked like ovarian cancer.
What did the reports reveal?
Her CA 125 level was 585 which is high but not alarmingly high. The CT suggested a dermoid cyst which is usually non cancerous. There was also bowel wall thickening near the ileocecal junction and nodules in the mesentery which hinted more toward abdominal tuberculosis than cancer.
Is every ovarian mass cancer?
No. Ovarian cancer at 41 is rare. Evening fever is not a cancer symptom. CA 125 can also rise in non cancerous conditions like endometriosis pelvic infections liver problems pancreatitis and tuberculosis.
What happened during laparoscopy?
When we did diagnostic laparoscopy we found multiple small nodules all over the peritoneum and omentum along with the ovarian mass. Samples were taken from the fluid peritoneum omentum and ovary for biopsy and TB testing. The ovarian dermoid cyst was removed carefully without rupture.
What did the final biopsy confirm?
The ovarian mass was a benign dermoid cyst and the peritoneal and omental deposits were due to tuberculosis. The patient started anti tubercular treatment and recovered completely. Her fever stopped she gained weight and her overall health improved.
What can we learn from this case?
Not all ovarian masses are cancer. Not all peritoneal nodules are metastasis. Sometimes abdominal tuberculosis can mimic ovarian cancer. A thorough evaluation and laparoscopy help find the right diagnosis and prevent unnecessary cancer treatment.
