Globally, 60–70% of ovarian cancers are diagnosed at Stage 3 or 4. Stage 3 means the cancer has spread from the ovaries to lymph nodes or the peritoneum, called peritoneal metastasis. Treating this effectively requires a protocol that targets both visible and microscopic cancer cells.

What is HIPEC?
HIPEC stands for Hyperthermic Intraperitoneal Chemotherapy. In this therapy, chemotherapy drugs are circulated directly inside the abdomen at 41–43°C. This local treatment targets cancer cells that are too small to detect on scans.

Drugs Used in HIPEC
We commonly use Cisplatin, a platinum-based drug, at 75–100 mg/m², circulated in the abdomen for 90 minutes. Heat improves drug penetration into tumor nodules, increasing effectiveness. This is why HIPEC significantly benefits ovarian cancer treatment.

Surgery and HIPEC
HIPEC is not standalone. It is combined with cytoreductive surgery, which removes visible tumors. HIPEC works best when there is no visible disease or low-volume disease less than 2.5 mm nodules, targeting microscopic residual cancer.

Scientific Evidence
A trial by Dr. Van Driel, published in the New England Journal of Medicine, showed combining HIPEC with surgery adds 12–14 months of survival without major side effects. This proves HIPEC improves ovarian cancer survival rates.

When HIPEC is Most Effective?
HIPEC works best after 3 cycles of chemotherapy, which reduces disease before surgery. Evidence does not support upfront surgery followed by HIPEC.
