- The standard treatment for patients with newly diagnosed advanced stage classical Hodgkin lymphoma is two cycles of ABVD followed by de-escalation to AVD (omission of bleomycin) in PET2 negative patients (RATHL study) or escalation to BEACOPP for PET2 positive patients.
- Per the ECHELON study, brentuximab vedotin and AVD (known as A+AVD) has improved PFS and decreases the need for autologous stem cell transplantation based on reduced relapse rates compared to ABVD, making this a promising first-line regimen. However, one must consider different toxicity profiles, with more lung toxicity with ABVD and more cytopenias and peripheral neuropathy in A+AVD.
