91-year-old Pakistani male with no smoking history, now diagnosed with stage IV adenosquamous of left upper lobe with bone metastasis. Molecular studies are negative for ALK alteration, PD-L1 greater than 1%, but there was insufficient tissue for additional studies

Summary:
- Cell-free DNA (liquid biopsy) is recommended for next generation sequencing
- If results show EGFR mutation, recommend initiation of osimertinib.
If actionable mutation is found in liquid biopsy, there is no need to confirm with tissue biopsy.
